Or maybe...Skank is just **** around for his own entertainment. Because...if Skank were really who he says he is, he would want to eradicate Medicare, not just reduce it. So either Skank is really a socialist, or...Skank has been such an asshole parent to his children that he knows at the end, his own offspring would just let him **** die rather than help him out.
